Website Information 

Browsing the website does not require you to provide any personal information.  The contact form only collects information that you provide which may include your name, address, phone number, and email.  Using the contact form is completely voluntary.  The information you submit is only shared internally with Starseed staff in order to respond to your requests and questions.  

 

Sharing Information with Third Parties

Your personal information will never be shared with third parties except when required for legal purposes or investigations. 

 

 

Email Communication

Information you provide via email links and online forms may be shared only with Starseed staff in order to respond to questions and requests.  Email communication is not secure and confidential and we do not recommend sharing private information via this method.  Private and secure communication should go through your patient portal or by calling our office.  Email communication from us is limited to providing links to your patient portal, onboarding forms, and appointment information.  We may occasionally send emails regarding important clinic updates or to inform you of new products or services. You may opt-out of email communication at any time through your patient portal, at the bottom of each email, or by informing the office of your preferences. 

 

 

Personal and Health Information

Your private and personal information is completely confidential and securely stored via HIPAA compliant electronic health records software. The information contained in your health records belongs to you and you are free to request copies for yourself or to share with other health professionals or individuals of your choosing. Requests for information need to be received in writing and have your signature and date of signing. We will never sell, rent, trade or otherwise share your personal and private information unless required by law or with your prior written consent.

 

 

SMS Terms and Conditions

When you opt-in to receive text messages from Starseed Energetics LLC (DBA Starseed Acupuncture + Wellness), you are agreeing to receive text messages related to your patient account.  This may include information regarding updates or requests regarding your appointments, information related to your patient portal, billing information and inquiries, and other messages pertaining to treatment plan or protocols.  You can opt-out of SMS messages at any time through your patient portal, texting STOP to 605-390-2300 or replying STOP to any automated text messages you receive, or by contacting our office with your request.  You will receive a final text message confirming your request.  If you would like to opt-in again at any time, you may do so through your patient portal or by sending a request to our office.  If you have any questions or issues with the messaging system, please contact our office directly at 605-390-2300.  

 

Message and data rates may apply for any messages sent from us and to us.  Message frequency may vary based on individual needs.  Carriers are not liable for delayed or undeliverable messages. 

 

 

Contact and Billing Information

The personal contact information you share through your onboarding process or update through your patient portal or in-person including name, address, phone number, and email is collected and stored via HIPAA compliant electronic health records software system.  Your billing information including credit card number is also collected and stored via HIPAA compliant electronic records software.  Starseed Energetics LLC does not have access to your full credit card number and will never sell, rent, trade or otherwise share this information.
